  1. Provide an overview of the Federal Duals program for patients with Medicare and Medicaid;
  2. Discuss the state of Michigan partnership for Medicaid reimbursement with the Affordable Care Act
  3. Identify the Michigan Department of Community Health’s role in managing multiple chronic conditions for Medicaid recipients

This presentation will address the following content:

Review the ACA’s influence on determining state demonstration for the Duals Medicare and Medicaid initiative and state demonstration

Identify the specific states that have/are participating in the Federal Duals/Program

Review content on state partnerships with the Federal Government and how this impacts state Medicaid use

Describe the new Michigan Medicaid program and how this has been developed to support the complex care needs of patients with multiple chronic conditions
